All requested information is required to participate in the rewards program. Cardholders will earn 5% on all qualifying purchases. Non-qualifying purchases include but are not limited to discounted items, clearance items, sale and promotion items. Rewards will not be earned on sales combined with any other offer, discount, or promotion. Rewards are not redeemable until 35 days after they are earned on a qualifying purchase. Rewards may not be redeemed with any other offer, discount, or promotion, including but not limited to birthday and anniversary offers. Rewards are not earned on sales when using rewards as payment.
